REGULATIONS

ORGANIZATIONAL REGULATIONS

of the entity performing medicinal activities – medicinal entity of Giorno Dentale Spółka z ograniczoną odpowiedzialnością seated in Kraków, entered into the Register of Entities Performing Medicinal Activities in the form of a medicinal entity by the Governor of Małopolska, register book no. 000000277549

I. General Provisions

§ 1.

  1. The terms and phrases used in these Regulations written with a capital letter have the following meaning:

    1. Regulations – these organizational regulations within the meaning of Art. 24 of the Act on Medicinal Activity, regulating matters regarding the method and conditions of providing health services by the medicinal entity Giorno Dentale Spółka z ograniczoną odpowiedzialnością;

    2. Giorno Dentale Spółka z ograniczoną odpowiedzialnością – the medicinal entity managed by Giorno Dentale Spółka z ograniczoną odpowiedzialnością with its registered office in Kraków (ul. Feliksa Konecznego 8/8U, 31 – 216 Kraków), registered in the Register of Entrepreneurs of the National Court Register (KRS) by the District Court for Kraków-Śródmieście, XI Commercial Division, under KRS number 0001082754, REGON 527508851, NIP: 9452285945;

    3. Giorno Dentale – the treatment facility (zakład leczniczy) of the medicinal entity Giorno Dentale Spółka z ograniczoną odpowiedzialnością;

    4. Act on Medicinal Activity – the Act of April 15, 2011, on medicinal activity;

    5. Working Days – days from Monday to Friday, between 7:00 AM and 8:00 PM, and Saturdays between 7:00 AM and 2:00 PM, excluding public holidays defined in the Act of January 18, 1951, on non-working days.

  2. The Regulations define:

    1. the business name of the medicinal entity;

    2. the goals and tasks of the medicinal entity;

    3. the organizational structure of the treatment facility;

    4. the type of medicinal activity and the scope of health services provided;

    5. the place of providing health services;

    6. the course of the health service provision process;

    7. the organization and tasks of individual organizational units and conditions for their cooperation;

    8. conditions for cooperation with other entities performing medicinal activities;

    9. the amount of the fee for providing medical documentation;

    10. the method of managing the organizational units of the facility;

    11. the receipt and handling of complaints and claims.

II. Business Name

§ 3.

The business name of the medicinal entity is: Giorno Dentale Spółka z ograniczoną odpowiedzialnością. The entity may use the shortened name: Giorno Dentale Sp. z o.o.

III. Goal and Tasks

§ 4.

The primary goal of Giorno Dentale Sp. z o.o. is to conduct medicinal activity consisting of providing paid health services aimed at preserving, restoring, or improving health, and performing other medical actions resulting from the treatment process or separate regulations. The entity provides:

  1. specialized dental treatment services;

  2. diagnostic tests performed to diagnose health status and determine further dental treatment.

IV. Organizational Structure

§ 6.

  1. The treatment facility operates under the name: Giorno Dentale.

  2. The facility includes the following unit: Giorno Dentale Dental Clinic.

  3. Activity is carried out through the following units and workstations:

    1. Dental Clinic (resort code 1800);

    2. Registration Point.

V. Type and Scope of Services

§ 7.

The entity provides ambulatory health services in the following fields:

  • Dental Treatment (Surgery, Orthodontics, Periodontology, Prosthodontics, Pediatric Dentistry, Conservative Dentistry with Endodontics);

  • Imaging Diagnostics (X-ray/Radiology);

  • Medical Devices (Prosthetic items);

  • Health Promotion and Education.

VI. Place of Service Provision

§ 8.

Services are provided in Kraków at: ul. Feliksa Konecznego 8U/8, 31 – 216 Kraków.

VII. Process of Health Service Provision

§ 9-11.

  • Services are provided by medically qualified personnel on a paid basis.

  • Registration is available in person, by phone (+48 456 568 020, +48 530 802 015), or via email (dentalegiorno@gmail.com).

  • Patients in emergency states are admitted without registration and out of turn.

§ 12.

Patients must provide their PESEL number and a valid ID. Before treatment, patients or legal guardians must submit:

  1. Written authorization for medical documentation access (Annex 2);

  2. Consent for disclosure of health information/secrecy waiver (Annex 3);

  3. Consent for the specific health service/procedure (Annex 4).

X. Fees for Medical Documentation

§ 19.

Fees for providing copies or extracts of medical records are calculated based on a percentage of the average salary in the previous quarter:

  1. One page of extract or transcript: 0.0002;

  2. One page of copy or printout: 0.00007;

  3. Documentation on an electronic data carrier: 0.0004. Current specific amounts are listed in Annex 7.

XI. Management

§ 20.

The Manager of the medicinal entity is the Management Board (Zarząd) of Giorno Dentale Spółka z ograniczoną odpowiedzialnością. Their tasks include organizing personnel work, supervising activities, and handling complaints.

XII. Complaints and Requests

§ 21.

Complaints are accepted at the Registration Point or via email. The Management Board considers complaints within 14 days of submission. A "Register of Complaints and Requests" (Annex 8) is maintained.

XIII. Final Provisions

§ 22-24.

The Regulations consist of 8 Annexes. Any changes require the same form and procedure as the introduction of the Regulations. The Regulations enter into force on the day of their adoption by the Management Board.
